SQUIRT HOUSE PARENT AND PLAYER MEETING

• Teams and coaches will be announced at this mandatory meeting. All four forms must also be turned in at this time and rosters signed (in black ink). The four forms can be found on the registration page where you registered your player. They are: Medical History/Consent to Treat USA Hockey Waiver of Liability Participant Code of Conduct Parent Code of Conduct
